BacterIal Die-Off

When treating Small Intestinal Bacterial Overgrowth (SIBO), the rapid and substantial killing of bacteria can trigger what is known as a die-off reaction. This occurs when bacterial cells rupture their outer membranes during cell death, releasing a surge of microbial components into the body, including:


  • Lipopolysaccharide (LPS) - also known as endotoxin

  • Structural components of the bacterial cell wall

  • Genetic material - DNA and RNA

  • Stored exotoxins - toxins actively produced by bacteria


Of these, LPS is the most potent trigger. When released into circulation, it acts as a danger signal to the immune system, binding to specialised pattern-recognition receptors. The most significant of these is Toll-like receptor 4 [ TLR4 ], which detects LPS and initiates the release of pro-inflammatory cytokines, TNF-α, IL-1, and IL-6. [28] These cytokines can trigger the onset of following symptoms, but are not limited to:


  • Worsening of the existing symptoms 

  • Cramping and discomfort

  • Loose bowel motions

  • Reduced Appetite

  • Nasuea 

  • Anxiety or Irritability 

  • Headache

  • Tiredness

  • Muscle Aches or Joint Pain

  • Excess Sweating 

  • Mild Fever or chills

  • Light-headedness

  • Skin Rashes 

 

Although some individuals do not develop die-off symptoms, many do experience variable symptom intensity, generally resolving within two weeks of commencing treatment.

 

The magnitude of cytokine production increases with the amount of LPS and other bacterial components released, meaning more severe SIBO raises the risk of die-off. However, factors such as individual immune sensitivity and bacterial strains with more immunostimulatory LPS also influence the reaction. Therefore, patients indicating more severe SIBO will be prescribed die-off support for preventive measures. 

 

In all cases, close monitoring of the body’s clinical response throughout this process is recommended, with particular attention to signs and symptoms that may indicate the physiological burden associated with die-off activity is exceeding the body's current capacity for regulation and elimination. 

In the situation of experiencing die-odd symptoms the following is recommended:


For subtle symptoms


Continue the detoxification whilst ensuring there is no progression in symptom experiences 


For mild symptoms


Take the supplement Bactrex every second day, whilst continuing the Curcuminoid Ultra / Turmeric daily, if it has been scripted. 


Ensure adequate hydration, nutrient-dense foods and rest. If symptoms reside, attempt daily dosage and return to every second day if symptoms return.


For moderate symptoms


Take the supplement Bactrex every third day, whilst continuing the Curcuminoid Ultra / Turmeric daily, if it has been scripted. Ensuring adequate hydration, nutrient-dense foods and rest. 


If symptoms reside too subtle or absent, attempt dosage every second day. If symptoms remain subtle or absent, attempt daily dosage and return to every second day if symptoms return.


For pronounced symptoms


Cease Bactrex supplements for 3 days, return to taking the Bactrex every third day, whilst continuing the Curcuminoid Ultra / Turmeric daily, if it has been scripted.


If pronounced symptoms return on every supplemental exposure, reduce the supplemental dose by half and continue every third day. If symptoms reside too subtle or absent at a full dose every third day, attempt dosage every second day. If symptoms remain subtle or absent, attempt daily dosage and return to every second day if symptoms return.


Whilst ensuring adequate hydration, nutrient-dense foods, engaging in gentle movement and rest.
